By accessing and using the Corelia Health website at coreliahealth.com and/or engaging our home care services, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our website or services.
2. Services
Corelia Health provides professional in-home care services for seniors and individuals with special needs across Ontario and Alberta, Canada. Our services include but are not limited to:
- Elderly care and personal care assistance
- Companion care and respite care
- Skilled nursing care at home
- Hospital discharge and post-surgery recovery support
- 24/7 home care services
- Chronic condition management
- End-of-life palliative care
- Special needs support
All services are subject to availability, geographic coverage, and a personalized care assessment.
3. Eligibility
You must be at least 18 years of age to use our website or request services. By engaging Corelia Health, you represent that you are legally authorized to enter into a service agreement on behalf of yourself or the care recipient.
4. Client Responsibilities
As a client or authorized representative, you agree to:
- Provide accurate and complete health, contact, and emergency information
- Inform us promptly of any changes in the care recipient's condition or needs
- Ensure a safe and respectful environment for our caregivers
- Adhere to agreed-upon scheduling and cancellation policies
- Make timely payments for services rendered
5. Service Agreements & Care Plans
Upon engagement, a personalized care plan will be developed based on your needs assessment. This plan may be adjusted over time as care requirements evolve. Any significant changes to the care plan will be discussed and agreed upon with the client or their authorized representative.
6. Fees & Payment
- Service rates vary based on the type, frequency, and duration of care required
- A detailed fee schedule will be provided prior to the commencement of services
- Payment terms are outlined in the individual service agreement
- Some services may be eligible for provincial funding or private insurance reimbursement
Corelia Health reserves the right to adjust rates with reasonable notice.
7. Cancellation & Termination
Either party may terminate services with reasonable written notice. Specific cancellation terms, including notice periods and any applicable fees, will be detailed in your service agreement. Emergency or safety-related terminations may take effect immediately.
8. Limitation of Liability
Corelia Health strives to deliver the highest standard of care. However, to the fullest extent permitted by law:
- We are not liable for outcomes that are beyond our reasonable control
- Our liability shall not exceed the total fees paid for services in the preceding 12 months
- We do not guarantee specific health outcomes or recovery timelines
Our caregivers are not a substitute for emergency medical services. In the event of a medical emergency, please call 911 immediately.
9. Intellectual Property
All content on the Corelia Health website, including text, graphics, logos, images, and software, is the property of Corelia Health and is protected by Canadian and international copyright laws. You may not reproduce, distribute, or create derivative works without our prior written consent.
10. Website Use
When using our website, you agree not to:
- Use the site for any unlawful purpose
- Attempt to gain unauthorized access to our systems
- Submit false, misleading, or harmful information
